𝗛𝗲𝗿𝗲’𝘀 𝗲𝘃𝗲𝗿𝘆𝘁𝗵𝗶𝗻𝗴 𝘆𝗼𝘂 𝗻𝗲𝗲𝗱 𝘁𝗼 𝗸𝗻𝗼𝘄 𝗯𝗲𝗳𝗼𝗿𝗲 𝘆𝗼𝘂𝗿 𝘃𝗶𝘀𝗶𝘁.

– Fasting for FBS/Lipid Profile tests (10-12 hours).
– Pregnant? Consult before X-Rays.
– No bottled water during Drug Testing.
– Females: Schedule URINALYSIS 5 days before/7 days after menstruation.

Documents to Bring:

– A. Letter of Authorization from your company specifying required tests.
– B. Government-Issued ID: Passport (Required for OFW applicants), Driver’s License, UMID, PhilHealth ID, TIN ID, Postal ID, Voter’s ID, PRC ID, Senior Citizen ID, National ID, PSA Birth Certificate, Police Clearance.
– C. Three Passport Size Photos.
– D. Blue Point Ball Pen.

Attire:
Wear loose, comfy clothes. No rompers, tight outfits, or accessories. “NO SANDO/SHORTS/SLIPPERS” policy.
